The Upper Grand Family Health Team (UGFHT) offers the opportunity to join an innovative, community‑based primary health care team serving the Fergus, Elora and Arthur areas. We are looking for a talented team player to join us as we continue to provide a model of excellence in rural primary health care.
Position Summary
The Nurse Practitioner (NP) is a vital part of the UGFHT, working collaboratively with physicians and other interdisciplinary health care providers to advance the organization’s vision of improving the health of our community through equitable access to high‑quality primary health care services. In this role, the NP provides comprehensive, patient‑centred care across the lifespan, demonstrating advanced clinical expertise in the assessment, diagnosis, and management of acute and chronic conditions.
