Facilitating multidisciplinary work and liaising with key stakeholder

Managing complex caseload

Supporting transitions

Implementing and monitoring strengths based interventions

Being accountable for own work and delivering cost effective interventions within agreed budgets

The team covers Adults with a wide spectrum of needs including LD, Autism , Transitions and school leavers, PD

We are co-located with a very efficient team of health colleagues including nurses, SALTs, psychologists.
