Reporting clinically to the Professional Practice Chief, Social Work and administratively to the Manager, Health Services, works in interdisciplinary environment by providing psycho-social assessments and treatment for clients and families in the Renal Program in order to maintain and improve upon the client's quality of life. Responsibilities Performs psycho-social assessments and formulates an appropriate treatment plan by reviewing referral, gathering information about the client/significant others with respect to social, emotional, cultural, response to illness, coping abilities, environmental and financial situation. Provides clinical and ongoing supportive counselling services to clients/significant others regarding identified problems, adjustment to illness, personal concerns, lifestyle changes, treatment objectives, withdrawal from treatment and bereavement counselling as required; completes written report for inclusion into the client's h...
