Detailed Overview

Reporting to the Manager, Clinical Operations, the Social Worker assists residents and their families to adjust to the stress associated with chronic illness/disability, relocation, and institutional life. Responsibilities Completes and updates psycho-social assessments of residents and clients; participates in pre-admission assessments, as required. Provides counseling services to residents/clients, families, staff and groups; make referrals to appropriate community agencies, as required. Attends interdisciplinary care conferences and consults with others, both internally and externally in relation to resident and client services. Assists with the development, implementation and review of resident and client care plans. Records actions taken on appropriate resident and client health records.
