As a Nurse Practitioner, you will actively provide and coordinate primary health services for those facing homelessness and complex health issues. Working closely with a multidisciplinary team, your work will involve health promotion, mental health counseling, and harm reduction techniques. Key Responsibilities:
• Deliver primary care and mental health support on the Health Bus
• Collaborate with healthcare teams and community partners
