Job Description

The Service Access and Transitions Leader is the creative relationship-builder responsible for key elements of service access, including move-in and move-out from CONNECT and developing key relationships in the community and within the health region.

The role oversees:
Service Access and monitoring, including the implementation of goal attainment scaling;

Transitions to and from CONNECT (both publicly funded and privately funded bedrooms); and

Exploring other service potential.
This leader will work closely with the Vancouver Island Health Authority and other partners and funders to facilitate smooth flow and transitions into and out of CONNECT. They will also oversee the development of CONNECT’s private capacity, including building the key relationships with private funders and the ABI community to keep an eye on the health of our private capacity.
