This full-time position is available until approximately September 2027. In this role, you'll offer individualized pharmacotherapy and collaborate closely with healthcare professionals on medication management. You will also contribute to clinical trials, medication safety initiatives, and quality improvement projects.
Key Responsibilities:
• Develop and manage therapeutic plans with healthcare teams
• Customize patient medication therapies through assessment
• Deliver effective counseling on medication use
• Participate in quality improvement and drug evaluation processes
• Act as a resource for pharmacy students and residents
Requirements:
• Registered Pharmacist with the College of Pharmacists of BC
• Graduate of an accredited Hospital Pharmacy Residency Program
