Fraser Health is seeking a Registered Psychiatric Nurse Liaison focused on psychiatry and substance use. This role is vital for providing assessments and interventions in both inpatient and Emergency Department contexts.
Your responsibilities entail performing clinical psycho-social assessments and creating comprehensive care plans aimed at addressing substance use concerns. Collaborating with a dedicated interprofessional team, you will enhance patient outcomes through ongoing patient advocacy and resource navigation. Your expertise will support timely patient admissions and transfers.
Key Responsibilities:
• Conduct clinical assessments on patients in crisis
• Collaborate in developing and adapting care plans
• Provide support through individual and group counseling
• Promote effective patient flow and care transitions
