In this position, you'll report to the Nursing Manager, participating in assessments and ensuring the effective implementation of individualized care plans. The role involves not only direct patient care but also documentation and collaboration with community partners. Flexibility is crucial, as shifts will cover essential nursing services seven days per week.
Key Responsibilities:
• Conduct assessments and manage client care plans
