Job Description
Deliver exceptional care as a Registered Practical Nurse (RPN) within the Hospital to Home program. This role focuses on supporting patients transitioning from hospital to home, providing essential assessments and interventions.
As a Registered Practical Nurse, you will assess, plan, and deliver patient care according to Ontario’s nursing standards. Collaborate with patient care managers and interdisciplinary teams to ensure effective discharge planning and continuity of care. Your strong clinical judgment will enhance quality improvement initiatives, focusing on patient advocacy during their transition from hospital to home.
