Registered Nurse

The Registered Nurse (RN) Case Manager collaborates, coordinates, and delivers healthcare services, with expertise in advocacy and a patient- and family-centered approach. You will work with clients across the life span with various needs in the home setting. You must be willing to drive up to one hour from your home site to the clients’ residence during all seasons and at various times of the day. This position does have some physical requirements such as bending, lifting, twisting, walking, climbing stairs, and carrying. The lifting and carrying could include heavy items. This position requires on call duties. Responsibilities include assessing, planning, implementing, and evaluating client care, focusing on chronic disease management, health assessments, wound care, ADL support, placement in facilities, and palliative care.
