The organization delivers a broad range of services—including information, education, case management, psychosocial rehabilitation, housing, and primary healthcare—through inter‑professional teams that address social determinants of health using a recovery model.
Overview of the role
Provides outstanding primary care nursing services for marginalized clients who may have severe and persistent mental illness, across the lifespan. The position is community‑based.
Responsibilities
- Participate in the full scope of nursing services as a member of a clinical team for all clients.
- Function within the Mission United hub model in all daily encounters.
- Complete triage assessments.
- Perform comprehensive mental health assessments.
