Enhance community health as a Registered Dietitian with CBI Home Health in the Hamilton region. This role allows flexibility and a focus on person-centered dietary care for clients at home.
CBI Home Health is on the lookout for a skilled Registered Dietitian to provide nutrition support in home settings. You'll focus on helping individuals maintain their health and dignity through customized dietary plans. Emphasizing empathetic care, you’ll work autonomously while engaging with family members to ensure optimal nutrition.
Key Responsibilities:
• Create individualized dietary plans for home care clients
• Work with interdisciplinary teams for client care
• Assess nutritional needs and provide ongoing support
• Educate patients and families on dietary choices
• Travel within the assigned community area
