Provide transitional care to stable patients 19 years and older while they await Long Term Care placement. Engage in a collaborative multi-disciplinary health team enhancing patient outcomes.
In this physician role, you will focus on delivering medical services to patients who have been moved from acute care settings. You will work closely with nursing staff, occupational therapists, and other health professionals to provide comprehensive care tailored to individual needs.
