As a Physical Therapist with CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence, from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life. Patients will be located in this area: Melrose Park, Oak Park, Berwyn, Cicero, La Grange, Berkeley, Oak Lawn, Elmwood Park.
As a Home Health Physical Therapist, you will:
- Receive a Sign-On Bonus
- Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
