The salary range for this position is CAD $45.15/Hr. - CAD $56.17/Hr.

Role Description

The Occupational Therapist or Physiotherapist Case Manager works as part of the Home Health team at Evergreen Community Health Centre, Vancouver, BC. This role delivers case management services to clients experiencing complex care needs, including brittle support systems, family conflict, social, cognitive and/or physical functioning decline, and/or mood and behaviour issues. The case manager conducts assessments that consider psychological factors and the surrounding social environment to support physical and mental wellness and functional ability in home and community settings.
