This is an exciting position embedded in a dynamic and supportive interdisciplinary team that covers the North Shore from Deep Cove to Horseshoe Bay.
You will be a medical consultant for patients who lack access to short term/urgent primary care and have been discharged from the Emergency Department or hospital. The program focuses on rapid response, reablement and stabilization for the client.
You will work within a team that includes another NP, RNs, Physiotherapists, Occupational Therapist, Pharmacist, and other allied healthcare professionals. You will be closely collaborating with care providers from Lions Gate Hospital.
