Provide vital care as a part-time Medical Doctor with CANES Community Care’s Transitional Care Program in Brampton and Bolton. Support medically stable clients undergoing short-term care with your clinical expertise.
This role allows you to engage with clients on a part-time basis, helping to promote safe transitions and optimal recovery. Work collaboratively with nursing teams by overseeing patient care, conducting assessments, and addressing urgent concerns after hours. Your commitment will play a crucial role in ensuring high-quality support for clients and families.
