Support injured workers' recovery as a Medical Case Manager with ForzaCare. This role focuses on effective coordination between patients, healthcare providers, and insurers.
ForzaCare is looking for a proactive Medical Case Manager who will play a vital role in ensuring efficient care management for injured workers. Ideal candidates will be licensed nurses or certified rehabilitation counselors with experience in the field. You’ll liaise with various stakeholders to ensure all parties are aligned in supporting worker recovery and return to their jobs.
Key Responsibilities:
• Coordinate care among medical providers and stakeholders
• Accompany injured workers to appointments
• Develop and track customized recovery plans
• Communicate progress regularly with involved parties
• Ensure adherence to customer and compliance standards
Requirements:
• Must possess a valid RN or CRC license
