• Lead an integrated palliative care team within a designated neighbourhood, managing day-to-day coordination of services to ensure continuity of care, seamless processes, and optimal patient and employee experience.
• Collaborate with your team to assess, plan, and coordinate with patients, caregivers, and other care-team members to establish, review, and revise care plans that support patients to die in their preferred place of death, respecting patient wishes for MAID as requested.
