The Care Coordinator will be the primary care manager for a panel of serious mental illness/SMI members with varying risk and may be assigned other health plan populations as needed.
This position is responsible for completing Health Risk Assessments, Individualized Service Plans and linking members to needed services to ensure that the members are receiving the healthcare they need.
