Role Overview
The Home Care Intake Coordinator is responsible for processing patient referrals and guiding patients and families through a smooth start-of-care experience. You’ll contact patients to explain our services, gather required information, and help coordinate their home care needs from discharge through long-term care enrollment, keeping everyone informed throughout the process.
