Position Overview

Practice setting: In home health assessments

Schedule: ASAP - Ongoing Monday - Sunday 7:00 AM - 7:00 PM

Workload: 5 to 8 assessments per day (assessments typically last 45 to 75 minutes; last appointment is scheduled around 6:00 PM)

Ideal availability: 2 full days per week or more, including at least one weekday (weekend only availability is not accepted)

What You Will Do

Complete in home health and wellness assessments for health insurance plan members

Perform H&P style evaluations and medication review

Collect vitals and complete basic point of care screenings as needed (for example blood pressure, finger stick, urine sample) and leave a fecal kit for the patient to mail in
