• Conduct visits to meet patient care needs
• Deliver ethical, safe nursing care per guidelines
• Assess and document patients’ needs accurately
• Adjust care plans in response to patient feedback
• Foster patient relationships for effective engagement
Requirements:
• Registered as a Practical Nurse with CNO
• Must have a driver's license and vehicle
