Job Description
Join our Clinical and Prevention Services as a Diversion Case Manager. This full-time contract role focuses on preventing youth homelessness for ages 16-24 through community engagement and support.
We seek a dedicated Diversion Case Manager with a minimum of three years' experience working with at-risk or homeless youth. Your role will involve identifying safe housing alternatives, liaising with landlords, and coordinating with families. You will provide ongoing follow-up case management and advocacy to ensure stable housing for young individuals in need.
Key Responsibilities:
• Engage youth facing or at risk of homelessness
• Mediate with landlords and housing providers
• Offer short-term case management for housing stability
• Distribute emergency funding to assist families
• Collaborate with community partners for youth services
Requirements:
• BSW or equivalent education; MSW preferred
• Registered Social Worker in good standing preferred
