Job Purpose

The Social Worker works collaboratively with a multidisciplinary team and partners with many groups such as government organizations, non‑profit organizations, the private sector, and a multitude of community groups and other community organizations. The Social Worker helps to identify and improve the social determinants of health that impact the overall wellness of individuals, families, and the community, such as income, education, social environment, community inclusion, culture, health practices, employment and working conditions. The Social Worker uses a population health approach through individual supportive counseling, group work, education, health promotion, advocacy, chronic disease management and linking people to community resources and helping them navigate systems. The work is based in client and family centered care and is guided by community health needs assessments as well as requests/referrals from clients, families and the community.
