Family Physician - Community - Chilliwack - Chilliwack General Hospital

Overview

We are looking for a Family physician with advanced training in Care of the Elderly to join our team in Fraser East, affiliated with Fraser Health Authority.

Responsibilities

• Provide care to home health and homebound seniors in the Chilliwack community, including home visits and liaison with the home and community care team and the client’s family physician.
• Collaborate in an interdisciplinary setting; may contribute to transitional care (hospital to home) to improve outcomes and reduce readmissions.
• Participate in academic and teaching components of the Service and pursue a clinical faculty appointment with the University of British Columbia.

Qualifications

• Advanced training in Care of the Elderly (certification preferred)
• Strong collaborative interpersonal and inter-disciplinary skills
• Experience in Community Geriatric consultations
