Executive Director - Primary Health Care

Reporting to the Chief Operating Officer, the Executive Director of Primary Health Care will provide bold, visionary leadership to drive transformation and innovation across the organization’s access and patient affiliation strategies. This pivotal role will focus on improving equitable access to healthcare through development and sustainment of the province’s Patient Medical Homes (PMHs), developing sustainable patient affiliation models, and recruiting and retaining staff to achieve HPEI’s mandate of attaching all Islanders to Primary Health Care.

As a trusted advisor to the Executive Leadership Team, the Executive Director will work as part of a dyad leadership model with the Medical Director to shape system-wide initiatives, create and advance integrated care pathways, and lead efforts to optimize patient experiences.
