Support young adults battling eating disorders as a Part-Time Family Physician. Your expertise in medical monitoring will focus on the 17-24 age group and complex cases.
This role involves comprehensive assessments, treatment planning, and limited follow-up care, vital for effective patient outcomes. Collaboration with community healthcare providers is essential for managing patient care. You will also partake in integrated care meetings to enhance the quality of care for clients.
Key Responsibilities:
• Monitor the health of young adults and complex cases
• Prepare assessments and tailor treatment plans
• Develop discharge strategies to limit readmissions
• Work with Family GP/NP and hospital teams
• Organize follow-up care and necessary therapies
