Be a part of the Children's Health Program as a Discharge Planner RN in Halifax, NS. Your leadership will ensure timely and effective discharge planning for pediatric patients.
This position reports to the Manager of Inpatient Surgical Care/Child Life Services. As a Discharge Planner, you will lead the discharge coordination process, working closely with families and healthcare professionals across NSH zones. Your expertise in community resources and pediatric care will be critical in facilitating smooth transitions from hospital settings to home.
