Community-Based Continuing Care Assistant
Job Description
Deliver compassionate care as a Continuing Care Assistant in community health. Provide personal support and assistance with daily activities, ensuring clients achieve optimal well-being.
This part-time role focuses on addressing physical, emotional, and psychological needs within various settings, including homes and clinics. You will travel to multiple community health sites, requiring excellent organizational and communication skills for effective client interactions. Independent work is fundamental to this position.
Key Responsibilities:
• Assist clients in personal care and daily living
• Foster emotional and psychological wellbeing of clients
• Provide support that promotes independence
• Communicate effectively with clients and families
• Engage in community travel as required
Requirements:
• Completion of Continuing Care Assistant certificate
• Effective organizational and interpersonal abilities
