Overview
As a key member of the multidisciplinary Country Health Connect team, you will provide a comprehensive community and inpatient based Diabetes Program in the Limestone Coast region. You will develop health promoting strategies for people with diabetes, coordinate the Diabetes Program, and liaise with other health professionals within the Limestone Coast Local Health Network (LCLHN) to ensure the highest quality evidence-based care for clients. This will include undertaking diabetes education and nursing practice and using primary health care strategies for individuals and groups.
