Case Manager for CTO Program
Job Description
Help transform lives as a Full Time Case Manager with CMHA in Toronto, dedicated to the Community Treatment Order Program. This role focuses on enabling recovery for those with serious mental health challenges.
Working with clients aged 16 and older, you will implement individualized plans within a psychosocial rehabilitation framework. Experience in culturally competent and trauma-informed care is valuable as you support clients in navigating their treatment and recovery.
Key Responsibilities:
• Implement and monitor individualized service plans
• Guide clients in adhering to CTO commitments
