Case Manager / Case Worker Part Time- West Ave (Hamilton)

Schedule and Compensation

Hours: Permanent part-time – Friday and Saturday, rotation of days, afternoons and overnights.

Rate of Pay: $24.89 per hour.

Responsibilities

• Develop and maintain a caseload.
• Complete an intake and assessment on each resident.
• Design, implement, in consultation with supervisor, an interview schedule that will provide an evaluation of quality of life improvement of successful program participants.
• Maintain confidentiality with regards to clients/staff and Centre's records.
• Observe all Good Shepherd health and safety rules and work in compliance with the provisions of the Occupational Health and Safety Act and its regulations to minimize risk to clients, staff and others as well as to the property of the Good Shepherd Non‑Profit Homes.
