Why this role is important?

The Nurse Case Manager will respond to patient inquiries, maintain a patient caseload, utilize effective communication skills, provide problem‑solving abilities both in a team environment and independently. Building strong relationships internally and externally is key to meeting team and business objectives. The role also involves attending pre‑booked training sessions, completing self‑learning assignments, and adhering to SDM Specialty Health Network SOPs, Policies, and College of Nurses Standards of Practice.
