Case Manager, Ambulatory Care

Sinai Health is looking for a Case Manager to support our Ambulatory Care at Hennick Bridgepoint Hospital.

In this role you will: Provide leadership and oversee the coordination of care and services for patients within the Day Treatment and Ambulatory Care programs Collaborate with and manage the interdisciplinary team to ensure comprehensive, patient-centered care from pre-admission through community follow-up Manage the referral system for the Ambulatory Care Program and clinics, including reviewing referrals and communicating decisions to internal and external partners Participate in and contribute to Quality Improvement initiatives, projects, and committees to enhance program effectiveness and patient outcomes Engage and collaborate with key stakeholders to ensure accuracy and integrity of Program Performance Metrics Chair and manage program weekly rounds Engage in scheduling and staffing initiatives to ensure operational efficiency Work in partnership with the Clinica...
