Job Description
Join FSC Woodlands as a Social Worker focusing on case management and community outreach. This role prioritizes client assessments and addresses the needs of various vulnerable populations.
As a Social Work Executive, you will manage a wide range of low to moderate complexity cases, participate in networking, and lead community outreach initiatives. The position requires thorough assessment processes and active collaboration during case conferences, alongside a commitment to proper documentation and evaluation procedures.
Key Responsibilities:
• Manage 25-30 active cases each quarter directly
• Conduct intake assessments based on practice standards
• Refer clients to appropriate external agencies
• Document and report on case outcomes timely
• Participate in programme team for group work initiatives
