Join CANES Community Care as a part-time Physician and enhance the Transitional Care Program in Brampton and Bolton. Your expertise will ensure safe transitions for medically stable clients in need of short-term support.
In this role, you will work approximately one day per week, focusing on providing medical oversight with a client-centered approach. Collaborate with nursing staff on patient assessments and ongoing evaluations to promote recovery and continuity of care. Your contributions are vital for facilitating smooth discharges back to the community.
Key Responsibilities:
• Conduct weekly rounds at Brampton or Bolton locations
• Review nursing staff prepared client updates
• Conduct virtual consultations as necessary
• Provide after-hours support for emergencies
• Manage referrals to specialists and aftercare services
