Sault Area Hospital is hiring a part-time Social Worker (BSW) to enhance the Geriatric Emergency Management Program. This role supports holistic assessment and care for older adults in the ED.
As a crucial addition to the GEM team, you will utilize validated tools for functional assessments, provide emotional support, and assist with the care planning process for elderly patients. Collaborating across disciplines, you'll advocate for patient-centered outcomes and help navigate community resources.
Key Responsibilities:
• Conduct psychosocial assessments for elderly in the ED
• Identify relevant social determinants of health
• Support families during health crises
• Participate in interdisciplinary care planning discussions
• Manage referrals to community and inpatient services
