Enhance senior nutrition care as a Consulting Dietitian with Seasons Care in Brockville. This role focuses on personalized dietary planning and nutritional support in long-term care settings.
Seasons Care Dietitian Network, a leader since 1996, is hiring for a part-time Consulting Dietitian working 30 hours per week. Your responsibilities will involve conducting nutritional assessments, developing care plans tailored to residents, and engaging in educational outreach to families. Collaborate with the interdisciplinary teams to ensure effective dietary interventions and foster a supportive environment for seniors.
Key Responsibilities:
• Monitor residents' nutrition and hydration levels
• Complete RAI-MDS assessments for new and existing residents
• Develop individualized dietary care plans for each resident
• Facilitate educational sessions for dietary staff on policies
• Participate in interdisciplinary care conferences
