Key Responsibilities:
• Select and onboard patients from a structured waiting list
• Deliver preventive and continuous care to your patients
• Manage a range of health conditions effectively
• Perform thorough assessments; develop and implement care plans
• Ensure preventive measures and vaccination are up to date
Requirements:
• CPSO certificate or ABFM certification required
