Adults Commissioning Reviews Social Worker – Impact & Outcomes

Connect2Hackney is looking for experienced Adults Social Workers to join their Commissioning Reviews Team on a locum basis. This role is pivotal in ensuring that commissioned services deliver high-quality, cost-effective care, improving outcomes for adults.

You will conduct person-centred reviews, manage complex cases, and collaborate with multiple agencies. The position requires a qualified Social Worker with at least 2 years of experience in commissioning reviews, strong knowledge of the Care Act, and an ability to undertake face-to-face assessments.
