OVERVIEW
The Administrative Coordinator, DFCM & Integrated Care plays a pivotal role in supporting the expanding integration, system-planning, and operational coordination of the portfolio of the Program Medical Director for Family & Community Medicine (DFCM), whose portfolio includes medical leadership of the Integrated Care, Transition and Palliative Care programs at MGH. The role provides coordination and administrative support to the Program Medical Director’s whole portfolio, ensuring alignment between hospital-based transitional initiatives, community primary care partners, and integrated care pathways.
The Administrative Coordinator will also provide high-level scheduling, coordination, documentation, and operational support for the portfolio work that intersects with East Toronto Health Partners Ontario Health Team (OHT) initiatives, particularly those related to primary care including work of the Primary Care Network (PCN) and Primary Care Act...
