Position Description

Reporting to the Manager, Transition Management, the Transition Facilitator is accountable for coordinating seamless transitions across inpatient areas to the community. The facilitator provides guidance and mentorship to create efficient and effective transitions. As a key member of the inter‑professional team, the facilitator works collaboratively with healthcare professionals, patients, and families to support patient‑centered transitions.
